What is Low Dose Naltrexone?
Naltrexone has been used medically for decades.
At conventional doses, it is an opioid-receptor antagonist used in the treatment of alcohol and opioid dependence.
Low Dose Naltrexone is different.
LDN describes the use of naltrexone at a fraction of the conventional 50mg dose, commonly in the region of 0.5mg to 4.5mg, although the appropriate dose is determined individually by a prescriber.
At these lower doses, researchers have investigated whether naltrexone may have effects relevant to pain signalling, endorphin activity, neuroinflammation and immune regulation.
LDN is now being investigated and prescribed privately for a range of conditions.
However, it’s important to understand that these uses are generally off-label, and the amount and quality of clinical evidence varies significantly between conditions.

Conditions for which LDN is being explored
Research and clinical interest in Low Dose Naltrexone covers a growing range of chronic conditions.
These include:
Fibromyalgia
LDN has been studied as a potential treatment for symptoms associated with fibromyalgia, particularly chronic widespread pain.
Small clinical trials and more recent systematic reviews have produced encouraging findings, although larger high-quality trials are still needed.
Long COVID
Long COVID can involve persistent fatigue, cognitive problems, pain, sleep disturbance and other symptoms following COVID-19 infection.
LDN has attracted research interest as a potential treatment for some people with Long COVID, but evidence remains emerging and it is not an established or licensed treatment for Long COVID.
ME/CFS – Chronic Fatigue Syndrome
Myalgic Encephalomyelitis/Chronic Fatigue Syndrome can involve profound fatigue, post-exertional malaise, sleep disturbance, cognitive difficulties and pain.
LDN is sometimes prescribed off-label for people with ME/CFS, but clinical evidence remains limited and it should not be presented as a proven treatment.
Mast Cell Activation Syndrome – MCAS
Mast Cell Activation Syndrome is associated with inappropriate activation of mast cells and release of chemical mediators.
There is clinical interest in the potential role of LDN in patients with MCAS and related disorders. However, evidence is currently limited and LDN is not licensed specifically to treat MCAS.
Inflammatory Bowel Disease – IBD
Inflammatory Bowel Disease includes Crohn’s disease and ulcerative colitis.
LDN should not replace established IBD treatment and anyone considering it should remain under appropriate medical/specialist care.
Small studies have investigated naltrexone at low doses in Crohn’s disease and other inflammatory bowel conditions.
Hashimoto’s Disease
Hashimoto’s thyroiditis is an autoimmune condition affecting the thyroid.
LDN is sometimes discussed as an adjunctive off-label treatment in autoimmune thyroid disease, but evidence for clinical benefit is limited.
It does not replace thyroid hormone replacement such as levothyroxine where that treatment is required.

The fact that LDN has been researched, discussed or prescribed for a condition does not mean that it is proven to treat that condition or that it will be suitable for you.

HOW DOES LDN WORK?
Naltrexone blocks opioid receptors.
At conventional doses, this receptor-blocking action is used clinically in alcohol and opioid dependence.
At much lower doses, researchers have proposed several mechanisms that could potentially explain the effects reported with LDN.
These include:
Temporary opioid receptor blockade
Brief receptor blockade may be followed by changes in endogenous opioid activity — the body’s own endorphin system.
Neuroinflammatory signalling
Research has investigated whether naltrexone may influence inflammatory signalling involving microglial cells in the nervous system.
Immune modulation
LDN has also been investigated for potential effects on immune and inflammatory pathways.
These mechanisms remain areas of ongoing research.
It is therefore more accurate to say that LDN may influence these pathways than to claim that it definitively “resets the immune system”.
Off Label Prescribing
Is Low Dose Naltrexone licensed in the UK?
Naltrexone is an established prescription medicine.
However, using specially prepared low doses for conditions such as fibromyalgia, Long COVID, ME/CFS, MCAS, IBD or Hashimoto’s generally represents off-label prescribing.
Off-label prescribing is lawful in the UK when an appropriately qualified prescriber determines that it is clinically appropriate.
It does mean that LDN hasn’t gone through the same regulatory approval process for each of these specific uses as a medicine specifically licensed for that condition.
Your prescriber therefore considers your individual circumstances before deciding whether treatment is appropriate.
Important Safety Information
LDN and opioid medication
This is particularly important.
Naltrexone blocks opioid receptors and can interact significantly with opioid medicines.
You must tell the prescriber about all medication you currently use, including occasional painkillers.
This includes prescription, over-the-counter and non-prescribed medicines.
